Condensation or other drainage not disposed of according to law. Observed water dripping from walk in cooler condenser onto shelving and food containers.

Raw animal food stored over/not properly separated from ready-to-eat food. In walk-in cooler, raw cold smoked salmon over cheese and vegetables. Operator moved to bottom shelf.
Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Two way splitter added, missing vacuum breaker opposite of green hose.
No chemical test kit provided when using sanitizer at three-compartment sink/warewashing machine or wiping cloths. No quaternary test kit available.
Operator is not properly tracking/marking the number of days opened commercially processed, ready-to-eat, time/temperature control for safety food was held at refrigeration temperatures prior to freezing in order to properly date mark the food when it is thawed and held at refrigeration temperatures again. In front display reach-in cooler, per operator cheesecake, cream filled macarons and tiramisu with cream purchased, repackaged and frozen, not properly marked with thaw day to track time, also per operator not more than 5 days. Spoke with operator regarding date labeling system.

Pesticide-emitting strip present near food prep area, by triple sink near clean dishes.
Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. On front counter by crepe makers, crepe batter with milk (74F - Cold Holding); pink crepe batter with milk (68F - Cold Holding) for two hours per operator. Operator states batters have been out of refrigeration often due to very busy service. Discussed with operator using time as a public health control and a four hour discard for items that must stay in constant use during busy production. Provided operator time as a public health control form. Operator placed batters back in reach-in cooler after completing orders.

Employee began working with food, handling clean equipment or utensils, or touching unwrapped single-service items without first washing hands. Observed employee enter into restaurant and kitchen from outside and begin work with no handwash. Spoke with employees, employee washed hands before donning gloves to begin work.
Handwash sink not accessible for employee use at all times. Floor cleaner blocking handwash sink in back of kitchen. Operator removed machine clearing sink for proper use.
Menu does not identify which items contain raw or undercooked animal foods covered by the consumer advisory. Smoked salmon per operator served raw, not marked as raw on menu.
Pet dog permitted in outdoor dining area without local ordinance allowing pet dogs in outdoor dining areas. Observed pet dog at outdoors table, no ordinance provided at time of inspection.

Time/temperature control for safety food thawed in an improper manner. Observed cooked ground beef thawing at room temperature, no running water. Operator placed thawing beef under running water in triple sink.
