Employee failed to wash hands before putting on gloves to initiate a task working with food. Employee touched face/glasses and donned gloves without first washing hands. Operator had employee stop, wash hands, and donned another set of gloves.
Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. No vacuum breaker after splitter installed at mop sink. Manager located breaker and installed during inspection.
