Carbon dioxide/helium tanks not adequately secured. Upon inspection, observed carbon dioxide tanks not secured in rear of establishment. Operator placed cable around tanks during inspection to secure the tanks adequately.
Equipment and utensils not properly air-dried - wet nesting. Upon inspection, observed storage pans on rack to be wet nesting in dishwashing area at establishment.

Warewashing sanitizing solution exceeding the maximum concentration allowed. Upon inspection, observed 800 ppm sanitizer concentration in sanitizing sink in dishwashing area when measured with establishments test strips. Manager added water to sanitizer and 200 ppm reading was observed.
Handwash sink not accessible for employee use at all times. Upon inspection, observed trash can in kitchen area blocking access to handwashing sink. Manager moved trash can so handwashing sink was accessible during inspection.
Equipment and utensils not properly air-dried - wet nesting. Upon inspection, observed storage pans wet nesting on dish rack in dishwashing area of establishment.

Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Automatic vacuum breaker missing from hose bibb at mop sink, and missing at pump next to dumpster.
Warewashing sanitizing solution exceeding the maximum concentration allowed. 3 compartment sink quaternary sanitizer reading 800 ppm, per manufacturer instructions, exceeds allowed concentration. Manager diluted sanitizer to 150 ppm.
Ice buildup in reach-in freezer and/or walk-in freezer. Ice build up in right corner of walk in freezer.
