Buildup of food debris/soil residue on equipment door handles. Observed multiple reach-in cooler, reach-in freezer door handles with dried debris. Staff was directed to clean these.
Dishmachine not washing/rinsing properly. Must wash, rinse and sanitize all dishware, equipment and utensils in three-compartment sink until dishmachine is functioning properly. Observed : the hi temp dish wash machine hot 197 on the rinse cycle.

Sanitizer bucket stored with food, equipment, utensils, linens, single-service, or single-use articles. Observed : sanitizer wipes tested at 200 ppm for quaternary ammonia stored on the ice maker bin. Removed wipes and put with cleaning.

Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed: the soft serve in the hopper on the front line was at 49°. A bag of ice was added and the unit was reset, and the compressor kicked on.
Floor tiles missing and/or in disrepair. Observed a chipped tile in front of the soft serve machine.
Sanitizer bucket/pouch stored with food, equipment, utensils, linens, single-service, or single-use articles. Observed: a sanitizer wipe dispenser bag stored on a counter with single serve items in the kitchen. The sanitizer tested at 300 ppm. The bag was moved below.

Food being cooled by nonapproved method as evidenced by inadequate rate of cooling during time of inspection. Observed: cooling chicken on the prep table at the cook line, 91°. 10 minutes later the chicken had only cooled 2°. The chicken was placed in the walk-in cooler, in which it dropped 9° in 11 minutes.
Wash solution in spray type hot water sanitizing dishmachine not reaching proper temperature according to manufacturer's specifications. Observed: the dish wash machine surface temperature were 146,154,159 in succession.
Buildup of food debris/soil residue on equipment door handles. Observed reach-in cooler handles in the prep area with dried food debris.
Employee personal food not properly identified and segregated from food to be served to the public. Observed a partial box of donuts stored on the back prep table over food for the public. These were discarded.

Hot water sanitizing dishmachine final rinse temperature at manifold exceeds 194 degrees Fahrenheit. Observed: the digital gauge reads 202° on the final rinse.
Floor area(s) covered with standing water. Observed: standing water around the mop sink. This was squeegeed away into the mop sink and a floor fan was put into action.

Handwash sink not accessible for employee use at all times. Observed: the back corner hand wash sink was blocked by an open case of bread on the floor. The case was moved to a shelf.
Bulk milk dispenser with dispensing tube longer than one inch protruding from the chilled dispensing head. Observed: a shake mix dispenser tube cut longer than 1 inch, on the front line.
Floor area(s) covered with standing water. Observed: standing water on the floor by the office. A floor fan is on in that area.
Food stored on floor. Observed: a case of bread/buns on the floor. This was moved to a shelf.
Standing water in floor drain/floor drain draining very slowly. Observed the drain under the 3 compartment sink drains slowly.
