Single-use gloves not changed as needed after changing tasks or when damaged or soiled. Observed employee touch cellphone then proceed to handle clean food equipment at steam table prep area without changing to new gloves. Employee changed gloves and washed hands.
Employee beverage container on a food preparation table or over/next to clean equipment/utensils. Observed employee drink cup on prep counter in main kitchen. Employee removed.
Employee wearing jewelry other than a plain ring on their hands/arms while preparing food. Observed employee with watch on wrist engaging in drink preparation at front counter.

Employee handled soiled equipment or utensils and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. At kitchen, Employee wiped hands on clothes with gloves on then proceeding to touch ready to eat food without changing gloves and washing hands. Advised operator of proper handwashing. Employee removed gloves, washed hands, changed gloves.
Reach-in cooler interior/shelves have accumulation of soil residues. At kitchen, thawing reach in cooler, visibly soiled with food debris.
