Single-use gloves not changed as needed after changing tasks or when damaged or soiled. Male employee wearing gloves and handling cooked container of chicken cleaned soiled surface (with raw chicken residue) using one hand and immediately after placed cooked chicken on Hot holding equipment with dirty hand. Manager addressed situation, employee removed gloves and washed his hands.
Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. At the cook line, cut tomatoes 51F manager moved to walk-in cooler.
Toxic substance/chemical improperly stored. Sanitizer stored on top of gloves at the raw chicken station, employee moved.
Paper towel dispenser at handwash sink not working/unable to dispense paper towels. Handwash sink near walk-in cooler entrance, manager fixed.

Employee failed to wash hands before putting on gloves to initiate a task working with food. Observed : female, red shirt cook line staff, adjust hairnet, then pull gloves to place on her hands, no hand wash. The manager spoke to the staff.
Bathroom facility not clean. Observed : in the men,s restroom, accumulation of paper on the floor.
No Heimlich maneuver/choking sign posted. Observed : lack of a visible sign. One was emailed.
Unwashed fruits/vegetables stored with ready-to-eat food. Observed : in the walk-in cooler, lemons in a bag on the upper shelf stored over, bagged lettuce. The lemons were moved to the bottom shelf.

Employee failed to wash hands before putting on gloves to initiate a task working with food. Employee on cook line picked up wrapped containers from floor, changed gloves without washing hands and proceeded to prep sandwiches. Manager had employee wash hands and change gloves.
Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. Item held hot in warmer on shelf above make line: sausage (118F - Hot Holding). Manager stated item placed in warmer 20 minutes prior to temperature being taken. Manager voluntarily discarded sausage.
Food-contact surface soiled with food debris, mold-like substance or slime. Water nozzles of tea machines next to kitchen ice machine.
No soap provided at handwash sink. No hand soap at hand washing sink next to kitchen ice machine. Manager provided soap to hand washing sink.
