Employee failed to wash hands before putting on gloves to initiate a task working with food. Employee changed gloves without washing hands after touched raw chicken on cook line. Discussed with operator and he washed hand properly before putting new gloves on.
Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. sliced cheese (48F - Cold Holding); sliced tomato (47F - Cold Holding); cut lettuce (49F - Cold Holding); shredded cheese mix (49F - Cold Holding) in reach in cooler by front line. Operator stated items are in that cooler about 1 hour. The items was stored in walk in cooler before putting out there. Operator removed the items to walk in cooler to cool down,
