Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed chicken at breading station (46F - Cold Holding). Chicken was double panned. Person in charge states it was out less than 4 hours but voluntarily disposed of chicken.
No proof provided that food employees are informed of their responsibility to report to the person in charge information about their health and activities related to foodborne illnesses. Some food employees hired less than 60 days not informed of their employee reporting. Provided employee reporting agreement and discussed with person in charge.

Observed ice scoop holster on drive through line soiled with black/green mold like substance. Black/green mold like substance was NOT in contact with ice scoop. Person in charge cleaned black/green mold like substance with sanitizer wipe.
Old food stuck to clean dishware/utensils. Observed old food stuck on out side of food containers that are stacked on green rack.
