Observed sliced provolone cheese, house made pasta salad (47F-48F - Cold Holding) in reach-in cooler across from conveyor oven on cook line. Operator stated employees were in and out of cooler a lot earlier. Operator removed both foods and placed in nearby reach in freezer to chill. Rechecked temperature at end of inspection cheese at 44F-45F and pasta salad at 41F
Food-contact surface soiled with food debris, mold-like substance or slime. Observed multiple soda nozzles on self dispensing soda machine soiled. Also, white deflector plate in ice machine soiled. Also, conveyor oven on cook line has accumulation of food debris.
In-use utensil stored in standing water less than 135 degrees Fahrenheit. Observed two utensils next to food warmer store in standing water of 77F. Operator emptied out water.
Single-service articles improperly stored. Box with wrapped sleeves of single service to go lids stored on floor next to storage area near restrooms. Also, box with single service lids for to go containers stored on floor on front service line. Operator removed wrapped lids to properly store.

Employee stated items were placed in cooler about an hour ago.Requested for items to be chilled or removed to working reach-in cooler but has not been moved as of the end of this inspection.
