Equipment in poor repair. The gasket on the left hand door of the reach in cooler in front of the triple sink and on both doors of the reach in cooler next to the steam table is damaged.

Raw/undercooked animal food offered and establishment has no written consumer advisory. Raw animal foods must be fully cooked prior to service. There is no consumer advisory for undercooked eggs on the menu. Inspector printed the operator the consumer advisory, operator hung the advisory to be visible to the public.
Duct tape used to repair nonfood-contact surface. The door to the ice machine has been repaired with duct tape.
Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. There is a employee cellphone and tablet on the food contact area of the slicer. Operator removed the employee personal items.
