Operator ran dishmachine 3 times, all tests were 0ppm chlorine. Operator states this morning he tested the machine and the ppm was 50-100ppm. Discontinue use of dishmachine for sanitizing and set up manual sanitization (3-compartment sink) until dishmachine is repaired and sanitizing properly. 3 compartment sink 200ppm quaternary

Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. In flip top cooler on cookline: cut lettuce (48F - Cold Holding) operator state's temperatures were taken at 10:30am, over 4 hours ago. Lettuce was not prepared today and has been in the unit since 9am. All other foods at proper temperature. See stop sale
Required employee training expired for some employees. To order approved program food safety material, call DBPR contracted provider: Florida Restaurant and Lodging Association (SafeStaff) 866-372-7233. EJ trained 2019, Andrew trained 2/11/2020, Wally trained 2018 Operator had pictures of the roster sent during inspection. Operator states there is an issue with the roster, as Wally was hired 2 months ago and the roster states he took his training in 2018. Discussed the need to have an up to date/accurate roster easily available at time of inspection.
