Dishmachine chlorine sanitizer not at proper minimum strength. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Dishmachine tested at 0ppm chlorine. Manager adjusted machine and chemicals and retested to 75ppm chlorine.
Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. Grilled chicken in warmer in kitchen at 125F. Per employee, chicken prepared 20 minutes prior. Operator decided to cool chicken and chicken was moved to the walk-in cooler. Temperature approximately 20 minutes later at 58F.
Vacuum breaker missing/incorrectly installed at mop sink faucet or on fitting/splitter added to mop sink faucet. Vacuum breaker installed at top of added splitter to mop sink. Operator moved vacuum breaker to non-chemical side of the splitter.
Drain cover(s) missing. Drain cover missing under back prep table. Employee located drain cover and replaced.

Cutting board has cut marks and is no longer cleanable. White cutting board on storage rack in dish room has deep cuts and no longer smooth.
Ice buildup in reach-in freezer and/or walk-in freezer. Ice build up in reach in cooler below salad dispenser in kitchen.
No conspicuously located ambient air temperature thermometer in holding unit. Multiple cold holding units in kitchen missing ambient thermometers.

Multiple reach in coolers in kitchen with black substance around exterior of unit where door closes. Plating around soda nozzles on soda machine on server line is soiled. Gasket on far cooler in the bar is soiled.
Reach-in cooler interior/shelves have accumulation of soil residues. Multiple reach in coolers in kitchen have accumulation of debris on shelving and interior of units. Interior top area of chip warmer on server line has build up.
