Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. At prep area, hose bibb with splitter on wall by prep sink missing backflow preventers on both ends.
No paper towels or mechanical hand drying device provided at handwash sink. At bar, no towels or soap at handwash sink. Operator supplied.
Carbon dioxide/helium tanks not adequately secured. In outside storage room, tank not secured. Manager placed chain around tank.
