Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Observed : the mop sink has a splitter, and the non chemical side lacked a vacuum breaker. One was added. **Corrected On-Site* ALSO, the auxiliary air conditioner units are water cooled. Their connection is at the mop sink with an added splitter. Because these are a hose connection, they require back flow devices. 2 more vacuum breakers were placed nearby to add on.
Carbon dioxide/helium tanks not adequately secured. Observed : the pressurized tanks by the #2 walk-in cooler were not secured. A strap was added.
Cold holding equipment not maintained in good repair. Do not store time/temperature control for safety food in this unit until the unit is repaired. Observed : the first reach-in cooler on the cook line had only house ranch cups in it. These were at 52°, having been prepped 2 days before.
Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed : the hinge support on the first reach-in cooler on the cook line has an accumulation of food debris.

Employee failed to wash hands before putting on gloves to initiate a task working with food. Employee placed a raw hamburger on grill, changed gloves then proceeded to handle a to go container and ready to eat food for hamburger without washing hands. Manager had employee wash hands and change gloves.

Employee stated hamburger placed in grill line drawers at 3 hours 30 minutes prior to temperature being taken. Manager had employee voluntarily discard hamburgers. Items stored in reach-in cooler next to cook line grill: chicken (45F - Cold Holding); sausage (44F - Cold Holding); shredded cheese (45F - Cold Holding); sliced cheese (44F - Cold Holding). Employee stated items placed in cooler 3 hours and 30 minutes prior to temperature being taken. Employee placed items in reach-in freezer to reduce temperature to 41F: chicken (41F - Cold Holding); sausage (40F - Cold Holding); shredded cheese (41F - Cold Holding); sliced cheese (42F - Cold Holding).

ALSO, on the cook line table top condenser; salsa 52, cut tomato 48, cut leafy greens 49.cooked onions 49. These items were discarded by the manage.
Dishware/utensils not soaked prior to being placed in three-compartment sink/dishmachine and results in soiled dishware/utensils after washing occurs. Observed: cutting boards coming through the dish wash machine, still soiled.
Employee beverage container on a food preparation table or over/next to clean equipment/utensils. Observed multiple lidded drinks throughout the kitchen over or next to clean dishes/food. Each one was moved below.
