Buildup of food debris/soil residue on equipment door handles. Observed microwave door handles soiled on cook line. Operator asked employee to wipe down.
Equipment in poor repair. Observed reach in cooler at cook line not functioning properly. As per operator, technician was called and they're coming to fix or replace.
Ice buildup in reach-in freezer and/or walk-in freezer. Observed ice buildup at reach in freezer where ice cream is helped next to office. Observed small build up of ice in walk in freezer.
Old labels stuck to food containers after cleaning. Observed on dry rack old labels on containers. Operator put into sink to be washed and removed.
Silverware/utensils stored upright with the food-contact surface up. Observed in dry storage area forks face up. Operator changed order and inverted them.
Wall soiled with accumulated grease, food debris, and/or dust. Observed soiled wall near prep sink with dust.

Reach in cooler in front of the flat top grill: marinara sauce (49F - Cooling). As per operator, prepped the day before, placed inside the walk in cooler, and put inside the reach in cooler the next morning.

Handwash sink removed from food preparation/dishwashing area. Must be reinstalled in the same location where removed. hand washing sinks: Bar area and the take out area by the drink machine.
No plan review submitted and approved - renovations were made or are in progress. Must submit plans and plan review application to DBPR H and R Plan Review office located at 2601 Blair Stone Rd., Tallahassee, FL 32399-1011. Plans must be submitted AND approved within 60 days. Observed the approved plans do not match the current layout of the establishment. The establishment has added a crew/party room nearby the exit door by the restrooms ( 48 additional chairs and 8 tables), the wall by the bar has been removed, the hand washing sink in the bar area has been removed ( observed pipes capped off in the wall where the hand washing sink was located) and by take out area the hand washing sink was removed.
