No soap provided at handwash sink. At the cook line hand sink. Soap was provided during the inspection
Spray bottle containing toxic substance not labeled. Under the dishwasher, spray bottles of degreaser and all purpose cleaner were not labeled. An employee labeled the bottles.

Dishmachine chlorine sanitizer not at proper minimum strength. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Dishwasher (Chlorine 0ppm) ran the dishwasher machine 3 times. All three times the reading was 0ppm Chlorine. Triple sink set up for sanitizing.
Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. At the front counter, butter packets (77F - Cold Holding). The item has been out of refrigeration for less than 3 hours. The butter was returned to the refrigerator to chill and cold hold

Raw animal food stored over/not properly separated from ready-to-eat food. In the reach in cooler, raw eggs were stored over mashed potatoes. The eggs were moved.
Handwash sink used for purposes other than handwashing. At the bar, the hand wash sink was used to refill water pitchers.
Commercially processed reduced oxygen packaged fish bearing a label indicating that it is to remain frozen until time of use no longer frozen and not removed from reduced oxygen package. On the triple sink, filets of corvina thawing in the ROP package. The chef removed the fish from the packaging.
Food stored on floor. In the kitchen, a bucket of rice was stored on the floor. The bucket was moved off of the floor.
Time/temperature control for safety food thawed in an improper manner. On the triple sink, shrimp, corvina, squid and guacamole were thawing submerged in standing water. The chef placed in running water to thaw properly.
