Incorrect chemical test kit provided for measuring the concentration of the sanitizer solution used in the three-compartment sink. Operator moved shrimp has test kit for quaternary sanitizer but is using chlorine sanitizer.
No proof provided that food employees are informed of their responsibility to report to the person in charge information about their health and activities related to foodborne illnesses. Operator had blank copy of form posted but no signed copies. Informed operator they must print and sign more copies.

Observed cut and marinated beef and chicken, dumplings, and cooked pork in walk-in cooler and reach-in freezer Employee stated the above items had been prepare onsite more than 24 hours prior.
Food stored on floor. Observed tub of shrimp in walk-in cooler stored on floor below shelves. Operator moved shrimp to shelf.
