Records/documents for required employee training appear to be falsified. Date of training and date of expiration have been changed on certificates for Shusheng T and Shukang T with the year appearing to be erased and written over.

Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Raw pork on kitchen table at 51F. Per manager, pork has been out of refrigeration for approximately 30 minutes. Pork moved to walk in cooler until oven was available 15 minutes later. Pork placed in oven to cook.
Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Interior of ice machine has black mold-like substance.

Ceiling/ceiling tile shows damage or is in disrepair. Ceiling tile in unisex restroom in disrepair with signs of water damage. Fan guards in walk in cooler are dusty. Wall along back exit by three compartment sink is soiled.
Exterior door has a gap at the threshold that opens to the outside. Back screen door threshold is not effective, light seen coming through at bottom of door.
Food stored on floor. Multiple cases of oil and sauces on floor in kitchen. Bag of rice on the floor by walk in cooler and bag of onions and bucket of sauces on the floor in the walk in cooler. Employees began moving all from the floor.

Plumbing system in disrepair. Pipe in unisex restroom ceiling is dripping into a bucket.
Stored food not covered. Prepared egg rolls in walk in cooler not covered. Frozen chicken in back reach in freezer not covered or wrapped. Employee placed all chicken in food bags.
