Observed guacamole (50/47F - Cooling); shedders cheese (48/46F - Cooling); cut lettuce (47/45F - Cooling); covered while was in cooling process, located in walk in cooler. Operator uncovered it.

Employee failed to wash hands before putting on gloves to initiate a task working with food. Observed a food handler removed her gloves and put on new gloves without washing her hands. The manager stopped her and had her wash her hands.
Clean glasses, cups, bowls, plates, pots and pans not stored inverted or in a protected manner. Observed the ice bucket not inverted. The manager inverted it.
Employee beverage container on a food preparation table or over/next to clean equipment/utensils. Observed an open cup of soda above bagged to go orders. The manager discarded the soda cup.

Observed 1 live fly land on a bag of napkins, 1 live flying insect in the ware wash area, 1 live flying insect flying by the entrance of the kitchen to the dining room, 4 live flying insects flying nearby the prep table in front of the cookline, 4 live flying insects on a shelf with containers of honey, vinegar, and sugar in the prep area nearby the fryer in the kitchen, and approximately 25+ live flying insects on the shelves with clean pans, and approximately 15+ live flying insects on the shelf and clean lids by the dishwasher.
Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Front counter: cut lettuce (67F - Cold Holding); cut spinach (64F - Cold Holding) as per operator, placed on the line 1 hour before the inspection. Observed the unit not on. Operator turned on the unit and placed the food items individually the walk in cooler.
Box containing roll plastic wrap has old food debris/soil residue inside. Observed in the prep area, two open boxes of Saran wrap.
