Equipment and utensils not washed, rinsed and sanitized in the correct order in three-compartment sink. Do not use dishes/equipment not properly sanitized. Observed employee rinsing before washing the containers, however, not rinsing again before placing the containers in the sanitizer. Educated the operator to compliance by rinsing after washing the containers.
No waste receptacle installed at handwash sink provided with disposable towels. Observed no waste receptacle at cookline handwash sink. Operator placed trash can near the handwash sink.

Warewashing sanitizing solution exceeding the maximum concentration allowed. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Dish machine at 200+ ppm for chlorine. Observed 3 compartment sink with reading of 400 ppm for quaternary solution.
No written procedures available for use of time as a public health control to hold time/temperature control for safety food. Did not observe a written plan for time/temperature control.
Bathroom facility in disrepair. Observed mens room out of order due due to toilet not flushing. Advised General manager to hang a sigh on ladies room for co use.
Drain cover(s) missing. Observed drain cover missing under 3 compartment sink and hot water draining and splashing all over floor. Observed General Manager getting a drain cover from storage and covering drain.
Employee chewing gum while preparing food or while in a food prep/food storage/warewashing area. Observed employee chewing gum at cash register, then went to food line to prepare customer food while chewing gum.
