Observed : water leak occurring from the ceiling, it has not rained today, nor is it raining now. 3 buckets are out catching the drips. A work order has been placed according to the manager on duty.

Sewage/wastewater backing up through floor drains. Observed : at a floor drain next to the ice machine, the ice machine was in drain mode, floor drain was overflowing on the tile floor. The 3 compartment sink was being dumped at the same time. The water receded shortly thereafter, the drain was cleared of any debris, and a test bucket of water was dumped. This ran easily through. Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. Observed : a rice cooker on the cook line, rice at 116°. The manager chose to discard.
Food-contact surface soiled with food debris, mold-like substance or slime. Observed : the plastic wrap holder with an accumulation of dried food debris in it.
Floor area(s) covered with standing water. Observed : water accumulation on the floor in the dining area.

Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. Observed: cooked chicken on the front line at 118°. This had been left on the counter after cutting. The chicken was then spread on the flat top until over 166°.
Employee with ineffective hair restraint while engaging in food preparation. Observed: front line staff, preparing food, hair tied at the scalp, yet the hair was long enough to swing past the shoulders when moving quickly. The manager coached said staff, and hair was then effectively restrained.
Single-service articles not stored inverted or protected from contamination. Observed: 3 stacks of lids on the soda area I;the dining room are contact side up.
A return visit. The problems from the previous inspection had been dealt with.
