Records/documents for required employee training do not contain all of the required information. Multiple food handler training certificates are missing the date of birth.
Dumpster rusted out on bottom. Bottom back of shared dumpster is rusted out with gap showing. Operator states the dumpster company has already been notified. Also, cove molding is disconnected from the wall under the lobby soda machine.
Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Employee cell phone on prep table across from flat grill. Operator coached employee and employee stored cell phone appropriately.
Floor/wall/ceiling/vent soiled/has accumulation of debris. Wall and floor near the baseboards under the three compartment sink has black substance.
Open dumpster lid. Right dumpster lid is open on shared dumpster.

Dishmachine chlorine sanitizer not at proper minimum strength. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Dishwasher (Chlorine 0ppm). Employee had to refill sanitizer due to bottle being empty. Manager rechecked at 100ppm.
Required employee training expired for some employees. To order approved program food safety material, call DBPR contracted provider: Florida Restaurant and Lodging Association (SafeStaff) 866-372-7233. Three employees food handler training expired. Charles Q. expired 1/16/26. Kiana J. expired 11/12/25. Arianna W. expired 1/14/26.
Equipment and utensils not properly air-dried - wet nesting. On clean dish rack next to dish machine, multiple metal pans stacked before properly air drying.
Floor, wall and/or ceiling soiled/has accumulation of debris. Floor under reach in coolers/ice machine across from cook line soiled with food debris Also, floor in walk-in cooler soiled with food debris.
Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Gaskets on reach in coolers soiled with food debris. Also, interior of warmer units on cook line soiled with food debris.
Open dumpster lid. On shared dumpster, right lid open. Manager had employee shut lid.
Single-service articles stored under leaking water lines. Under soda machine counter at drive thru, cardboard box of plastic to go lids stored under dripping line from soda machine. Manager cleaned box and put tray under dripping line.
Standing water in bottom of reach-in-cooler. Standing water in reach in cooler at drive thru window. Manager stated work order already in for cooler.

No proof provided that food employees are informed of their responsibility to report to the person in charge information about their health and activities related to foodborne illnesses. One employee hired less than 60 days, per manager did not have employee responsibility agreement signed. Emailed manager DBPR Form HR 5030-103.

Employee handled soiled equipment or utensils and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Employee working in dish area handled soiled containers, slid rack into dish machine, pulled clean containers out and handled clean containers without washing hands. Manager coached employee on proper hand washing procedures.
Employee touching ready-to-eat food with their bare hands - food was not being heated as a sole ingredient to 145 degrees F or immediately added to other ingredients to be cooked/heated to the minimum required temperature to allow bare hand contact. Establishment has no approved Alternative Operating Procedure. Employee prepping red onions with bare hand in front prep area. Onions are not being heated. Employee stopped, washed hands and placed gloves on.
Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. On front counter, pan of tofu 72F. Tofu placed out approximately 30 minutes prior. Tofu reheated 179F.
Bowl or other container with no handle used to dispense food. Paper bowl used as scoop for container of tortilla chips in drive through area. Bowl discarded.
Floors, walls, and/or ceilings soiled/has accumulation of debris. Wall under triple sink in kitchen (pipe as well).

Employee touched face/hair and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Employee rubbed eye with bare hand then continued to handle clean utensils on cook line without washing hands in between tasks.
Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. White rice (121F - Hot Holding); brown rice (108F - Hot Holding) in same section of hot wells at front counter, per manager both items where placed in well less then 1 hour prior, manager turned up temperature of hot well and voluntarily discarded both pans of rice.
Soap dispenser at handwash sink not working/unable to dispense soap. At hand wash station by dish machine, manager replaced batteries.
Employee wearing jewelry other than a plain ring on their hands/arms while preparing food. One employee at front counter making food orders wearing watch, employee corrected during inspection.
