In-use utensil in non-time/temperature control for safety food not stored with handle above top of food within a closed container. Observed a spoon handle not stored aboard dried oregano on the cook line. The operator properly stored the spoon handle.

Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed gaskets on standup coolers soiled with black mold like substance or slime. Observed employee cleaning gaskets.
Outer openings not protected with self-closing doors. Observed entrance door not self closing. Observed door wide open at entrance.

Bathroom facility in disrepair. Observed closure on door for ladies room, possibly bent to where it rubs on door causing hesitation in opening and closing of door.
Bowl or other container with no handle used to dispense food. Observed employee using a 4 oz to go container as a scoops, to scoop jalapeños.

Cutting board has cut marks and is no longer cleanable. Observed grooves in cutting boards that were stored clean next to dishwasher.
Reach-in cooler shelves with rust that has pitted the surface. Observed in reaching stand up cooler shelves starting to rust.

Observed: Floor not cleaned when the least amount of food is exposed. Observed a bunch of Ice on drain rack at soda station and all over floor, Soda company has been notified for repair on ice chute dispensing too much ice. Priority: Basic
Observed: Marked exit/path to marked exit blocked. For reporting purposes only. Observed back exit blocked with trash cans. Explained to operator that it can’t be blocked at any time. Operator moved trash cans so they are not blocking exit. Priority: Reporting

Soap dispenser at handwash sink not working/unable to dispense soap. Soap dispenser full, but batteries were dead. Observed General Manager Replacing batteries immediately in the mens room.
Bathroom facility in disrepair. Observed Ladies room closed, General Manager opened bathroom and the ceiling has a major water leak and ceiling damage going on. General Manager has submitted work repair. In the meantime, advised General Manager to put up signage labeling the mens room, Women's and Men's.

Cleaned and sanitized equipment or utensils not properly stored. Observed a dirty 1 cup scoop hanging up next to a clean 1/2 cup scoop. Observed manager removing said items and putting them into dish area.
Food stored on floor. Observed food on floor in walk in cooler. Observed manager correcting issue immediately.
