Time/temperature control for safety food identified in the written procedure as a food held using time as a public health control has no time marking. All items in reach in cooler near drive thru area being held on time while cooler is not working properly. No times written on log for today. Manager stated all items were stocked at 10:30-10:45 and wrote all items with times on log.
Toxic substance/chemical improperly stored. Spray bottle of bathroom cleaner hanging on side of bag- in- box soda rack near back door. Employee removed spray bottle and stored properly.
Floor soiled/has accumulation of debris. Floor under stand up reach in coolers in kitchen soiled with food debris. Also, floor under cook line tables soiled with food debris. Also, walk-in cooler floor soiled with food debris. Manager had employee begin to clean.

Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. At front line, steak (129F - Hot Holding). Cook stated steak was cooked 1 hour prior. Operator had employee put steak back on grill and rechecked at 168F before returning to warmer.
Handwash sink used for purposes other than handwashing. In handwash sink by back door, two cleaning brushes in sink. Operator removed. Also, in handwash sink next to cook line/front counter food debris in sink.
No paper towels or mechanical hand drying device provided at handwash sink. No paper towels available at handwash sink by back door. Operator stocked paper towels.
Required employee training expired for some employees. Equipment and utensils not properly air-dried - wet nesting. Multiple pans in dish area stacked before properly air drying.

Unwrapped single-service utensils not presented so that only the handles are touched. In lobby, unwrapped single service knifes not stocked with all handles facing up. Operator let employee know all handles need to be up.

Employee rubbed hands together for less than 10-15 seconds while washing hands. Employee using broom to sweep, went to wash hands after completing sweep, and washed hands for approximately 5-6 seconds. Manager coached employee on proper hand washing procedures. Employee washed hands correctly.
Hot time/temperature control for safety food received at less than 135 degrees Fahrenheit. Brown rice sitting on counter by cooking equipment 114F. Rice placed on counter approximately 20 minutes prior. Rice voluntarily discarded.
Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Breakers missing at the ENDS of three splitters at mop sink faucet (none needed for chemical hose).

Employee on grill dropped utensil on floor, picked up utensil with gloved hands, removed gloves and placed on new gloves without washing hands. Manager coached employee on proper hand washing procedures. Employee washed hands correctly.
Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Back flow preventer missing on top faucet at mop sink at the ends of three splitters (two have no hose attached, one has black hose attached). Inspector noted that hose to chemical box does NOT need breaker due to breaker present in chemical box.
