Certified Food Manager or person in charge lacks knowledge of foodborne illnesses and symptoms of illness that would prevent an employee from working with food, clean equipment and utensils, and single-service items. No big 6 poster. Manager could not list them all. Provided manager with big 6 poster, which she displayed.

Employee, Jason, hired less than 60 days ago with no verified training and had not signed employee responsibility form. Inspector provided manager with form and had employee sign.
Cutting board has cut marks and is no longer cleanable. Cutting board in drive thru area and prep line by grill grooved beyond being thoroughly cleaned. Manager advised they receive new cutting boards every month.
