Observed employee grabbed a paper sheet from the cook line floor and continued to serve a drink to a customer without washing his hands prior.

Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed coleslaw (50 F - Cold Holding), as per manager food was placed in ice less than 1 hour ago. Manager refilled the ice container to maintain proper cold temperature.
Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. Observed at hot holding unit at cook line spicy chicken (128 F - Hot Holding); spicy chicken (130 F - Hot Holding), as per manager put in the hot holding unit 35 minutes ago. Operator decided to discard the chicken.
No paper towels or mechanical hand drying device provided at handwash sink. No paper towel available in hand wash sink located by warewashing area, and also at hand wash sink cook line area.
