Observed the dish wash machine has been converted to lo-temp chemical. Tested at 0 ppm. Jug was empty. Replaced and achieved 100 ppm.
Food-contact surface soiled with food debris, mold-like substance or slime. Observed : the first soda gun in the bar is soiled.
Floor area(s) covered with standing water. Observed : standing water at the mop sink. ALSO, by the ice machine by the walk-in cooler. Staff started squeegeeing.

Hole in or other damage to wall. Observed : the corner and base cove tile are in disrepair at the entrance of the bulk soda room. ALSO, the 1 step up seating area has the top step flooring removed leaving an uneven transition point.

Raw sewage on ground of establishment. Observed : the dish wash machine floor drain was overflowing and running to the floor drain two feet away. A work order has already been placed, no one was in the dish pit when I stepped into the open end. The manager stated: they will stop dish wash machine operations, set up the 3 compartment sink, squeegee the floor dry, and then sanitize the floor, the area will then be blocked off until the plumbers come. The 3 compartment sink is now set up.

Hot water sanitizing dishmachine final rinse not reaching proper temperature at manifold. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired. Observed: the dish wash machine final rinse, highest measure was 158°. Manifold was 193.
Employee beverage container on a food preparation table or over/next to clean equipment/utensils. Observed an employee drink stored over the clean end of the dish wash machine. The drink was discarded.
Floor tiles missing and/or in disrepair. Observed the tiles in dry storage at the doorway are broken off.

Dishmachine not sanitizing properly. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Observed: the high temp dish wash machine gauge, final rinse temp reads 189° consistently. However, the irreversible thermometer only reads 124-131°. The rinse bar starts out with a strong flow and drops to just above a light mist within a few seconds of activation.
Food being cooled by insufficient method as evidenced by inadequate rate of cooling during time of inspection. Observed: pico made this afternoon, not dropping at least 1° per 8-9 minutes. An ice bag was added, and the cooling rate advanced to 3° in a 9 minutes span.

Handwash sink used for purposes other than handwashing. Observed: the hand wash sink on the prep line with multiple items in it. The items were removed.
