Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. Inside warmer in pizza prep area: cheese sauce (122F - Hot Holding). Manager stated the cheese was reheated on the stovetop and moved to the warmer approximately 30 minutes prior, then had an employee reheat the cheese to 177F during this inspection.
Toxic substance/chemical improperly stored. 1. Spray bottle containing chemical cleaner stored o table beside rolled silverware in server station. Manager removed the spray bottle from the table during this inspection. 2. Several cases of wine stored on floor directly below chemical storage shelf in storage room. Manager moved the wine to another area during this inspection.
Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Backflow prevention device missing at red hose connection after splitter added to mop sink.
Employee beverage container on a food preparation table or over/next to clean equipment/utensils. Employee drink cup on prep table near cookline. Manager removed the drink from the table during this inspection.
Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Employee purse stored on top of bag containing single-use plastic cups in bar area. An employee removed the purse from the area during this inspection.
Employee wearing jewelry other than a plain ring on their hands/arms while preparing food. One employee at cookline preparing and handling food with bracelet on wrist.
Hole in or other damage to wall. Gap in wall panel in back of kitchen, near water heater.
In-use tongs stored on equipment door handle between uses. Tongs hanging from oven door handle at cookline. Manager removed the tongs from the handle during this inspection.
Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. 1. Green wire shelving on dish rack soiled with dust. 2. Gaskets on cooler doors in bar area soiled with mold-like substance.
Outer openings not protected during operation and vermin and/or environmental cross contamination present. Screen doors in bar area left open during service. Several flies observed in kitchen and/or bar area. Inspector reviewed the original provisos with manager and the manager closed the screen doors during this inspection.

Operator moved items to ice bath to rapidly chill.
Operator is doing a special process, conducting reduced oxygen packaging, cooking time/temperature control for safety food to a time and temperature combination lesser than those in the Food Code or operating a molluscan shellfish tank for human consumption without a HACCP plan approved by the Division of Hotels and Restaurants.
