Raw animal food stored over/not properly separated from ready-to-eat food. In cooler near ice machine Steak in package was placed above blueberries. Raw pork chorizo over pooled eggs. In reach in cooler across from cook line, pooled eggs were placed directly over ready to eat raw salmon. Operator rearranged for proper storage.
Wiping cloth sanitizer solution exceeds the maximum concentration allowed. Sanitizer Bucket (Chlorine 200+ppm) on front line by entrance to kitchen. Operator diluted sanitizer in bucket at time of inspection. (Sanitizer bucket chlorine reading 100ppm).

Ice buildup in multiple reach-in freezers.
Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. There is an accumulation of dust in multiple places, the storage racks above clean dishes walk area, the fan in walk in cooler, and on the ice machine.

Employee touching ready-to-eat food with their bare hands - food was not being heated as a sole ingredient to 145 degrees F or immediately added to other ingredients to be cooked/heated to the minimum required temperature to allow bare hand contact. Establishment has no approved Alternative Operating Procedure. Observed Server remove toast from the toaster and began buttering toast using bare hands. Educated Server about bare hand contact with ready to eat food. Toast was discarded and remade with gloved hands.
No proof provided that food employees are informed of their responsibility to report to the person in charge information about their health and activities related to foodborne illnesses. Owner unable to locate during the visit. Left blank form with Owner, who had an emergency and needed to leave.
