Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed the following foods temperature greater than 41°F; cut lettuce (47F - Cold Holding); sliced turkey (49F - Cold Holding); cut melon (49F - Cold Holding); raw beef patties (47F - Cold Holding); cooked beef (47F - Cold Holding) All were located in walk in cooler 3 and 4. As per chef all foods were moved from reach in cooler to walk in cooler this morning. Advised chef that cooler had a ambient temperature of 49°F. Chef instructed employee to remove all foods and place in walk in cooler 1.

No plan review submitted and approved - renovations were made or are in progress. Must submit plans and plan review application to DBPR H and R Plan Review office located at 2601 Blair Stone Rd., Tallahassee, FL 32399-1011. Plans must be submitted AND approved within 60 days. Inside lobby Bar area removed, one of the kitchen walk in coolers removed and replaced with restroom, outside bar hand washing sinks relocated and outside bar area floor plan Changed. No plans with changes submitted to state.

Cardboard used to line food-contact shelves. Located on shelf with dry foods.
Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Observed AC vents soiled with accumulated dust. Located throughout kitchen area.
Cold holding equipment not maintained in good repair. Do not store time/temperature control for safety food in this unit until the unit is repaired. Observed walk in cooler 4 and 3 both with ambient temperature of 49°F. Advised chef, called maintenance and moved all foods to walk in cooler 2.
Commercially processed reduced oxygen packaged tuna, cobia and mahi bearing a label indicating that it is to remain frozen until time of use no longer frozen and not removed from reduced oxygen package.
Exterior door has a gap at the threshold that opens to the outside.
Food stored on floor. Observed various cases of food being stored on floor in walk in freezer. Chef moved to shelf.
