No handwashing sign provided at a hand sink used by food employees. Observed no hand washing sign at the hand wash sink in the walk in cooler, operator put a sign up.

Employee failed to wash hands before putting on gloves to initiate a task working with food. Observed employee with gloves on move his hat and touch his hair then went to the food prep area, stopped employee explained importance of washing hands and changing gloves, employee washed hands.
Pesticide/insecticide labeled for household use only present in establishment. Observed household pesticide in dry storage in back of kitchen.
Food-contact surface soiled with food debris, mold-like substance or slime. Observed Manuel can opener with food debris on the blade in the kitchen, employee cleaned it.
Commercially processed reduced oxygen packaged fish bearing a label indicating that it is to remain frozen until time of use no longer frozen and not removed from reduced oxygen package. Observed Yellowfin Tuna and Atlantic Salmon in the walk in cooler not removed from packaging and no longer frozen, operator has an employee removing from the packaging.
Cutting board has cut marks and is no longer cleanable. Observed a cutting board in the prep area that is grooved in the kitchen.
Equipment in poor repair. Observed reach in cooler in the middle of the kitchen the gaskets are torn on both sides.
Food stored on floor. Observed a container of sliced onions on the floor in the walk in cooler operator moved.
In-use tongs stored on equipment door handle between uses. Observed tongs on the door handle of the grill on the cook line.
Single-service articles not stored inverted or protected from contamination. Observed cones in the wait staff area in a tube not protected, operator covered.
Walk-in cooler/freezer shelves with rust that has pitted the surface. Observed in the walk in cooler some shelves are rusted and pitted.

Whole meat roast hot held at less than 130 degrees Fahrenheit. Prime rib left out to rest at 101 F. Operator placed back to cook, temperature was 125.

Establishment has no written procedures for employees to follow in response to a vomiting or diarrheal event where the vomit or diarrhea is discharged onto surfaces in the establishment.
Handwash sink not accessible for employee use at all times. Observed dish machine baffles laid over handsink at dishwash area.
Employee wearing jewelry other than a plain ring on their hands/arms while preparing food. Observed employee on expo with watch and bracelet on wrist. Observed Employee with multiple filigree rings handling open food plates.
