Nonfood-grade basting brush used in food. Observed a non-food grade brush used for butter spread. The operator took the brush out of service.

No waste receptacle installed at handwash sink provided with disposable towels in the back prep area of the kitchen. The operator placed a trash can near the hand sink.

Observed the dishmachine chlorine sanitizer at 0ppm after running 3 times. The operator set up the quaternary solution in the three compartment sink.
No soap provided at handwash sink in the kitchen.
Paper towel dispenser at handwash sink not working/unable to dispense paper towels in the kitchen.
Water with a temperature of least 85 degrees Fahrenheit not provided/shut off at employee handwash sink. Observed hot water at the handwashing sink behind the front counter at 71F. The operator turned on the hot water to 92F.
Clean equipment/dishware/utensils stored below soap or paper towel dispenser exposing the items to drips from people's hands. Observed clean glassware stored underneath a paper towel dispenser near the hand sink behind the front counter. The operator moved the glassware.
In-use wet wiping cloth/towel used under cutting board. The operator removed the wet cloth from underneath the cutting board.

Employee touching ready-to-eat food with their bare hands - food was not being heated as a sole ingredient to 145 degrees F or immediately added to other ingredients to be cooked/heated to the minimum required temperature to allow bare hand contact. Establishment has no approved Alternative Operating Procedure. Observed server getting a breakfast bread to heat in microwave with bare hands. Had server throw away bread, wash hands and use tongs.
Ceiling/ceiling tile shows damage or is in disrepair. Observed a large hole in ceiling in prep room. Observed some mesh on it, but it's still open.
Employee with no hair restraint while engaging in food preparation. Observed employee with no hair restraint on.
Vending machine exterior soiled. Observed door handles of reach in cooler with food debris build up

Cove molding at floor/wall juncture broken/missing. Observed cove molding in dining room leading to restrooms coming unattached.

Observed in house made cookies packaged and identified, but not date marked nor name and address of where it was made.
Opened employee beverage container in a cold holding unit with food to be served to customers. Observed employee drink in reach in cooler. Observed operator putting a labeled container for employees only to segregate items.
