Dead roaches on premises. Observed one dead roach on floor in corner of dry storage area behind main kitchen. Person in charge discarded. Put in work order for facilities.

Person in charge states has been on line for 2 hours. Recommend holding on time as items are not lidded. Will hold on time for remaining 2 hours.

Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. cut tomatoes, cooked chicken (50F-52F - Cold Holding) In cold bar on cook line. Operator stated items went in cooler about an hour ago. Operator removed both foods and placed in nearby cooler to chill.
Required employee training provided by a third-party employee training program and operator unable to provide original certificate for employees trained. Two employees certificates not original certificate.
Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Gaskets on reach in freezer on cook line soiled.

Observed contracted sushi service employee drop equipment on the ground, pick up equipment from ground, then return back to working at prep line without hand wash. Person in charge spoke to employee during inspection.
No mop sink or curbed cleaning facility provided at establishment. Establishment has no mop sink at the time of inspection. Has been removed from janitors closet. Are currently using floor drain.
Wiping cloth sanitizing solution stored on the floor. Sanitizer bucket on line stored on floor. Person in charge added second bucket.
