Handwash sink not accessible for employee use at all times. Observed a rolling storage rack blocking access to the handwashing sink near the three compartment sink. The operator removed the rack allowing access to the handwashing sink near sink.

Exterior door has a gap at the threshold that opens to the outside. Observed at back door by restrooms a gap on the bottom of the door

Wet mop not stored in a manner to allow the mop to dry. Observed wet mop that is used for restrooms, in a bucket by restrooms with mop head stored down. Observed operator remove mop and store correctly inverted with mop head up.

Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Observed ice bin with black like mold substance around lid.
Carbon dioxide/helium tanks not adequately secured. Observed in bar area next to keg cooler a Co2 tank not secured.
Clean glasses, cups, bowls, plates, pots and pans not stored inverted or in a protected manner. Observed forks in storage cup not inverted with handles up. Observed General Manager removing, re washing and storing handles up.
Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Observed personal cell phone in server area on counter next to reach in cooler. Observed General Manager removing cell phone to proper location.
Opened employee beverage container in a cold holding unit with food to be served to customers. Observed multiple drinks in reach in cooler at server station. Observed General Manager segregating items to bottom of cooler.
Oyster/clam/mussel/crab or other shells purchased from a supplier reused to serve food. Observed large white real mollusks shells being used to serve New England style stuffers, then being saved, and being washed for re use. Explained to General Manager that, that is not allowed and observed General Manager throwing them away.
