Commercially processed reduced oxygen packaged fish bearing a label indicating that it is to remain frozen until time of use no longer frozen and not removed from reduced oxygen package. Observed thawed tilapia sealed in reduced oxygen packages with label indicating to be removed from packaging prior to thawing. Manager removed tilapia from packages during inspection.

In-use utensil not stored on a clean portion of food preparation or cooking equipment. Observed clean tongs and spatula stored on dry soiled cloths in front of grill at cook line. Employee discarded soiled cloths during inspection.
Plumbing system in disrepair. Observed hand wash sink shut off in storage room. Per manager hand wash sink hot water was leaking and was shut off until repair.
Water leaking from pipe and/or faucet/handle. Observed pipe leaking into bucket under three compartment sink at kitchen area.
