Raw animal food stored over/not properly separated from ready-to-eat food. In the reach in cooler across from the cook line, observed raw live Mussels over cooked pasta. In the Stand alone cooler observed container of raw shrimp placed directly on top of container of cooked pork ribs. In the reach in cooler in the far left side of the kitchen observed, Raw tuna placed above ready to eat carrots and blue cheese. Operator rearranged foods for proper storage.

Handwash sink used for purposes other than handwashing. Observed individual dumping beer into the handwashing sink located at the bar. Operator stated individual was a contractor fixing their beer machine. Discussed with operator handwashing sink is only for handwashing to reduce and prevent contamination.

Establishment packaging time/temperature control for safety food using a reduced oxygen method that does not control the growth of and toxin formation of C. botulinum and the growth of Listeria monocytogenes. Establishment does not have an approved variance from the Division of Hotels and Restaurants. Observed Fresh Grouper and Salmon Reduced oxygen packaged in reach in cooler. Per operator, fish was just cut and packaged. Educated operator about requirements with the 2017 Food Code and reduced oxygen packaging.
Menu contains an item made with a raw/undercooked animal food as an ingredient and description of item does not inform consumer of raw/undercooked animal food ingredient. Smoked Salmon Flatbread offered on menu with no secondary designation, such as an asterisk to indicate consumer advisory applies.
No proof of required state approved employee training provided for any employees. Per operator, certificates were destroyed in ft.myers beach from flooding.
