Time/temperature control for safety food identified in the written procedure as a food held using time as a public health control has no time marking. Observed milk in thermos for coffee with no time mark. Per operator, out of temperature for approximately 2 hours. Operator elected to discard milk.
Toxic substance/chemical improperly stored. Observed Sterno fuel stored next to clean dish ware on clean dish rack in kitchen. Operator stored properly.

High temperature dish machine not reaching minimum of 160F using DBPR provided temperature strips. Advised operator to use triple sink for sanitizing dishes until dish machine is repaired.
