Observed in flip top cooler butter pc (44-45F - Cold Holding) stored in a pan and inverted in to another pan, per operator held out of temperature less than 3 hours operator placed in lower portion of flip top. Observed at coffee counter a pourer of 2 percent milk (45F - Cold Holding) out at room temperature. Per operator held out of temperature less than 4 hours. Operator will place on TPHC.
