Employee cracked raw shell eggs and then handled ready-to-eat food and/or clean equipment or utensils without washing hands. Observed employee touched the eggs mix with gloves, discarded the gloves and put on new gloves without washing hands. Operator was instructed that the hands need to be washed before put on new gloves.
In-use utensil stored in unclean water at or above 135 degrees Fahrenheit. Observed utensils inside dirty water at 128°F. Operator changed the water and started to reheat the water.
Raw animal food stored over/not properly separated from ready-to-eat food. Observed raw shell eggs stored next to lettuce at walk in cooler. Operator relocated the lettuce.
