Live, small flying insects found. Observed 4 flying insects in the following locations: 3 around bar and 1 in back kitchen.
Roach activity present as evidenced by live roaches found. Observed a total of 11 live roaches around salad reach-in cooler: 1 on floor under cooler toward back wall, 1 crawling on bottom vents of cooler, 3 crawling up walls on sides of reach-in cooler, and 1 on shelf holding reach-in cooler installed above sand which reach-in cooler. Operators were able to pull salad reach-in cooler out, kill roaches roaches along salad cooler and walls, and sanitize them, but unable to kill roach on shelf under reach-in cooler installed above salad cooler. Another 2 observed on floor and cove molding under electrical panels across walkway from salad reach-in cooler. Before end of inspection, observed another 1 in corner over salad reach-in cooler, 1 in outlet by drink machine, and other 1 on clean pan on drying rack in warewashing area.
Floor soiled/has accumulation of debris. Food debris under salad reach-in cooler. Operators moved cooler and cleaned up debris.
Plumbing system in disrepair. Hole in bottom of mop sink.
Unsealed concrete floor in food preparation, food storage, warewashing area or bathroom. Floor worn past seal throughout kitchen.
Water leaking from pipe and/or faucet/handle. Leak under ice bin at bar, under hand wash sink at bar, and under hand washing sink in front of fryers.
Worn, torn and/or soiled floors/carpeting. Floor worn throughout kitchen.

Equipment and utensils not properly air-dried - wet nesting. Upon inspection observed moisture between stacked plastic Pans on drying rack. Employee preceded to separate the pans during inspection..
Ice buildup in reach-in freezer and/or walk-in freezer. Upon inspection observed ice buildup inside walk-in freezer.
