Employee cracked raw shell eggs and proceeded to grab a clean plate to fixed customer's cooked food without first washing hands. Reviewed proper handwashing procedures. Employee washed hands correctly.
Raw animal foods not properly separated from each other in holding unit based upon minimum required cooking temperature. Trays of raw chicken stored above trays of raw beef meatloaf in walk in cooler. Employee inverted.

Dishmachine not sanitizing properly. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Observed high heat sanitizing dishmachine not reaching 160F at dish level. Operator contacted service company for repair. Operator set up three compartment sink, calculated, and manually measured out sanitizer to measure at (Sink and Surface Sanitizer - DDBSA/LA 700/1875) in the three compartment sink. Employees started manually sanitizing dishware and utensils and service technician is in route to service machine.
Sanitizer solution other than chlorine, quat or iodine not at the proper minimum strength listed in the manufacturer's instructions. Do not use equipment/utensils not properly sanitized. Observed Sanitizer Bucket Sanitizer Bucket (Sink and Surface Sanitizer - DDBSA/LA 848/2258) in prep area and coming out of dispenser at the three compartment sink. Operator contacted company for service. Operator calculated and manually measured out sanitizer and water and had sanitizer buckets corrected to (Sink and Surface Sanitizer - DDBSA/LA 700/1875).
