Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet: Establishment has a splinter on their mop sink faucet. One side of the splitter has a hose that goes up to a chemical dispenser. The other side goes to a hose used for cleaning the floors. This is the side that needs the back flow preventer.
Food-contact surface soiled with food debris, mold-like substance or slime: Soda gun on the right end of the bar is dirty. General Manager cleaned during the inspection.
Menu does not identify which items contain raw or undercooked animal foods covered by the consumer advisory: Consumer advisory is listed and marked on the menu. However, the food associated to it are not marked with a symbol.

Inspector test strips show no reading for chlorine. Triple sink with Quaternary ammonia set up for sanitizing dishware.
