In-use utensil in non-time/temperature control for safety food not stored with handle above top of food within a closed container. Bowl with no handle and plastic spoon inside sugar bin without handles above food.
A return visit. The problems were not fixed yet and the kitchen was given more time.

Dishmachine not washing/rinsing properly. Must wash, rinse and sanitize all dishware, equipment and utensils in three-compartment sink until dishmachine is functioning properly. Dishwasher is not sanitizing properly. Instructed operator how to set up three compartment sink.
Food stored on floor. Observed bag of onions on floor in walk in cooler, and raw chicken in plastic bin stored on floor in walk in freezer. Operator placed items 6 inches above floor during time of inspection.
