Ceiling tile missing. Observe in rear room with freezer.
Exterior door has a gap at the threshold that opens to the outside. Observe at rear door.
Hole in or other damage to wall. Observe large hole in wall by electrical panel.

No proof provided that food employees are informed of their responsibility to report to the person in charge information about their health and activities related to foodborne illnesses. Observed with employee with all employee are less then 60 days. I emailed operator and operator printed and had current employee sign form.
Working containers of food removed from original container not identified by common name. Observed one employee oil, water and yellow substance not labeled at cook line.
