Handwash sink used for purposes other than handwashing. Ice dumped into handwashing at kitchen entrance.
No proof provided that food employees are informed of their responsibility to report to the person in charge information about their health and activities related to foodborne illnesses. One employee working in kitchen without food handler training who has not signed the employee health reporting agreement. Manager stated she has been employed for six weeks and had the employee read and sign during this inspection.

Cutting board has cut marks and is no longer cleanable. Cutting board at front counter has deep cut marks and is no longer smooth and cleanable.
Single-service articles not stored inverted or protected from contamination. Box containing foam takeout containers stored directly on floor inside dry storage room. Manager placed box on a shelf during this inspection.
A return visit. The problems from the previous inspection had been dealt with.

Toxic substance/chemical improperly stored. Bottle of soap and bottle of sanitizer tablets stored on shelf beside clean dishes at cookline. Manager removed both from shelf during this inspection.
No plan review submitted and approved - renovations were made or are in progress. Must submit plans and plan review application to DBPR H and R Plan Review office located at 2601 Blair Stone Rd., Tallahassee, FL 32399-1011. Plans must be submitted AND approved within 60 days. Operator added two ice cream freezers, soft serve machine, reach-in cooler, prep tables, and a handwash sink. Operator has been in contact with Kay Clark and has submitted a drawing of the plans, but the plans have not yet been approved.

Unwashed fruits/vegetables stored with ready-to-eat food. Box of unwashed tomatoes stored above jug of milk and cans of whipped topping inside reach-in cooler behind ice cream counter. Employee moved box of tomatoes to the bottom shelf during this inspection.
