Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. In the left hand make table cooler chicken (47F - Cold Holding. Operator voluntarily discarded the chicken.
Accumulation of black/green mold-like substance in the interior of the ice machine/bin. There is a mold like substance around the ice chute in the ceiling of the ice machine. Dishwasher (Chlorine 0ppm), operator changed out the empty container of sanitizer. Chlorine sanitizer 100ppm.
Ice buildup in reach-in freezer and/or walk-in freezer. There is a.buildup of ice at the door of the walk in freezer.

No soap provided at handwash sink. At bar Hand washing sink observed no soap or paper towels. Operator replenished soap and paper towels.
Employee beverage container in ice machine/ice bin. Observed employee beverage in bar ice bin. Operator removed beverage and discarded affected ice.

Observed sliced cheese (50F - Cold Holding); cut tomatoes (51F - Cold Holding) in the reach in cooler nearest the walk in freezer. The operator stated the potentially hazardous foods had been held for approximately 3 hours. The operator placed a cover on the reach in cooler and will monitor temperatures.
Food-contact surface soiled with food debris, mold-like substance or slime. Observed the can opener soiled with an accumulation of food debris.

Employee beverage container on a food preparation table or over/next to clean equipment/utensils. Observed an employee beverage stored on a rack above single-service drink lids. The operator properly stored the beverage.
In-use knives stored in cracks between pieces of equipment. Observed in-use knives stored between a food preparation table and the Bain Marie. The operator moved the knives to the dish area.
Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed the surface surrounding the soda nozzles soiled with a green mold-like substance.
Ten (10) foot-candles of light at least 30 inches from floor not provided in walk-in cooler. Observed no lighting in the walk in cooler.

Observed dishwasher machine testing at 0ppm. Operator had already called technician. Instructed to set up three compartment sink to properly sanitize dishes.
Single-use gloves not changed as needed after changing tasks or when damaged or soiled. Observed employee not change gloves or wash hands upon returning after leaving cook line and back prep area to complete other tasks. Educated employees and operator on proper hand washing, employees washed hands before continuing to work.
Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Observed no vacuum breaker on hose bib at mop sink.
