Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Observed threaded hose Bibb on hot water tank without back flow device.
Handwash sink not accessible for employee use at all times. Observed employee hand washing sink blocked by a rolling cart. Employee removed cart to allow for hand washing.

Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. hot dogs (66F - Cold Holding) stored on counter next to cooking equipment. Employee stated he had the hot dogs out for only about 15 minutes. Employee placed hot dogs on the hot dog roller to reheat.
Wiping cloth sanitizer solution exceeds the maximum concentration allowed. Sanitizer Bucket (Quaternary 500+ppm) at front counter area and in prep area. Operator remade both sanitizer solutions and retested at 200PPM
Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Employee personal belongings stored on top of cases of bottle water in walk-in-cooler. Operator removed employees bags, and beverages to properly store.
Equipment in poor repair. Gasket on walk in freezer door torn.
