Observed employee engaging in food preparation touch beard guard with gloved hands and proceeded to engage in food preparation. Inspector spoke with manager regarding hand washing. Employee removed gloves and washed hands.
Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Observed employee phone stored on prep table. Employee removed and stored properly.
Employee wearing jewelry other than a plain ring on their hands/arms while preparing food. Observed employee engaging in food preparation wearing a watch. Employee removed watch.

Live, small flying insects in kitchen, food preparation area, food storage area and/or bar area. Observed 20 live flying insects on top of lid of shake machine which is located at far end of the wall on the front line for employee to serve from. Operator has voluntarily closed of this section which is only serving shakes and bottled drinks. Rest of establishment has no issues. Operator has pest control route to treat area. Operator has Saran Wrap entire shake machine and closed off area as well.
