Wiping cloth sanitizer solution exceeds the maximum concentration allowed. Sanitizer Bucket (Quaternary 500ppm); at back counter. Test strip turned blue which exceeded the 200-400 concentration that manufacturer has on chart. Employee added more water to dilute solution. Retested at 200ppm.
Floor tiles missing and/or in disrepair. Flooring/ diamond plate flooring in walk-in-cooler buckled in are near front door.
Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Walk-in-cooler gasket soiled.
