Time/temperature control for safety food reheated for hot holding not reaching 165 degrees Fahrenheit for 15 seconds within 2 hours. In steam wells in prep area, cooked beef chili (129F - Reheating); cooked beef gravy (127F - Reheating), both previously cooled from hot, reheating for 2 and a half hours per operator. Both were discarded.

Dishwasher handled dirty dishes then clean dishes, no glove change or handwash. Spoke with employee about proper handwashing techniques, employee changed gloves and washed hands before continuing work.
Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Dust accumulated on vent cover in walk-in freezer.
Open dumpster lid. Operator closed the lid.
Wiping cloth quaternary ammonium compound sanitizing solution not at proper minimum strength. Sanitizer Bucket (Quaternary 75ppm) across from dishwashing area. Operator replaced bucket, quaternary 300ppm.
